Fish got stuck in overflow box...

I had a perfectly healthy 1 inch convict tang get stuck in the overflow tower and somehow fied. Did he get stuck against the drain or what? What can I do to prevent this
 
it died and then got stuck to the overflow.
 
Was it physically stuck in the grate, or was the water flow just holding it there? If he wasn't stuck, I'd say he was either dead or very weak before he wound up there. Wouldn't think there would be enough water pressure to keep a healthy fish against the overflow. Sorry for your loss.

I see what you mean, but the water level is below the top of the grate and the only way in would've been jumping. I'm very surprised, he was my most active fish, aswell as the smallest. He was also my favorite

I had a scooter blenny Jump into my overflow weir, and did not realise he had gone missing till I did a routine maintenance in the overflow and found him dead. I do now have a white filter media cut to size that sits inside the weir not only does it act like a noise dampener you can see visual if any fish or critters manage to end on top of it rather then losing them down in the bottom. Sorry for your loss
