Fish help please

hi all, I have a question. I have 45 halo on jbj mixed reef. The tank is well established and cycled. All my numbers are good. Right now I have two captive bred clownfish and a Midas blend, and s flame hawk fish. My flame hawk is sick now and looking like he's not going to make it. I just added the blenny a week ago.. this has happened before. I've noticed that when I add a fish within a few weeks someone else dies. Am I doing something wrong? The other fish always seem to be fine.. it's like I add the new guy and the older fish dies.. I drop acclimate and everything.. should I add a few fish or just one at s time? Again all my levels are good just don't understand why a fish goes update a new addition?

With the way the supply chain for fish works these days, it's almost impossible to keep ich, velvet, Brooklynella, Uronema, flukes or a host of other nastiness out of dealers' tanks. Some try. (None around me, though...) A few are successful. A _very_ few are routinely successful. At the same time, the fish are being dropped into plastic bags full of stale water and their own wastes, they're being presented with strange little things they've never seen before, and all of their normal foods have vanished.

I assume that any new fish has flukes, velvet and/or ich. I treat for all of those routinely in QT before ever letting new fish into my display. Even that may not be good enough, as the resting stages of some of those baddies can ride into your display on a snail shell or frag plug.
