I will try to keep it short, I upgraded from two 25 Gal tanks to a 150 Gal tank Mixed reef About 4 months ago. Transferred the live rock and corals from the two tanks to the new tank over a week or so. Added fish from the old tanks after about two weeks (three clown fish ). Added rock that was cured in my garage over 4 weeks. All was fine no issues corals were happy fish were happy. Then we added 6 Cromis when about two months old, I added a Copper Banded Butterfly two weeks later along with a Yellow Eye Tang and a Blue Sided Anthis the Butterfly developed ICK, All signs of this went away after a few days. All fish were eating normal all seemed fine. A couple of weeks later I seen spots on the Tang and the Clowns.. Not long after all that was left was one Cromis and the Blue Sided Anthis. Sadly I did not have a top on my Tank and the Anthis jumped out..
So I didn't add anything to the Tank Cromis is still fine to this day. But About a week ago I added a Yellow Tang he lasted a week and died. this weekend I tried a Six Line Wrasse he only lasted two days and is now dead.

I feel I am missing something, Any Ideas ?
 
Your not missing anything but a little info. Ich is still in your tank. It will be until you let it go fallow (fishless) for 76 days. Here's more detail on what your dealing with and how to handle it. Please ask any questions you have after reading it!

Yes you have to decide if you are going to try and do Ich management or Ich eradication. If you just keep adding fish. You will just keep losing fish. Re set up those 25 gal tanks treat fish with copper or hypo. Leaving big tank fish less for 76 days.
 
I agree with melypr, your DT is infected. ICH can remain undetected in the gill tissue. In the life cycle, It's only visible for a few days. Fish can also build somewhat of an immunity to ICH. However, it will get exacerbated once the fish is stressed.
