Fish keep dying

I have had my tank set up for 3 months now and everything started fine until my clownfish started dying. I lost 2 black snowflakes, 2 Ocellaris and also 3 purple queen anthias. I have taken my 2 Picasso clowns to QT and discovered missing scales on their heads? All parameters are normal:
Ammonia: 0
Nitrate: 0
Nitrates: 10
Salinity: 1.025
Temp:79
In my DT I have 1 sailfin tang, 1 six line wrasse, 3 pajama cardinals, 1 coral beauty angel and 1 remaining Ocellaris. Any ideas why my fish (mainly my clowns) are dying or what the missing scales on the heads could be? Posting pics

Did you quarantine your fish before putting them in your tank?

I’d guess you have a case of flukes and maybe ich as well. It’s hard to tell with a mostly white clownfish. You will need to do a 5 minute freshwater dip to check for flukes. Do you know how to do that or do you need information on the procedure?
 
I did not, I got them from LFS. I did freshwater dip my 1 Picasso but nothing changed. I’ll have to research flukes.

Do the freshwater dip in a dark bucket or container and see if there is anything on the bottom of the container after you take the fish out.
 
I would suspect Brook or Uronema is a possibility by the list of fish you've lost.

Have there been anything on the fish other than the discolored areas?

Brook can be especially hard to see on the white part of a clown fish.
 
Do 6 line wrasses sometimes cause significant aggression? Supposedly to other 'peaceful' fish? Do you see mucus peeling off the fish - or just the (what appear to be) missing scales? (to me they look more like 'scrapes' unless im not looking at the right part - especially under the 'bluish' light.

Curious - over what time period did the losses occur? Were all of the clown pairs in the tank 'together' and then some started dying one by one - or was it you had a pair - they died - you replaced them with another pair etc etc. Did the clowns have any symptoms at all before dying?
