Flame hawk #2- drops dead

I have had two flame hawks, both have literally dropped dead within minutes. Let me begin, I fed the fish, everyone was fine, swimming around. Flame hawk eats, drops dead within a minute. That is the second time I have literally had one drop dead in front of me. No spots, no illness, the rest of the tank 100% fine? I thought the first time was complete coincidence... two now?

Anyone?
 
Following. I added a flame hawk to my tank last week, the next day he was gone. As in disappeared. And it's a new tank, the scape isn't so full of corals that he could be hiding somewhere.
 
LFS but months apart. Months of no issues and a healthy eating fish.
I'm thinking that your LFS is getting them from the same wild-caught source, and that there is a problem with their collection methods which causes trauma to the fish, long-term decline, and ultimately an early death.
