Flesh receding hammer?

What can I do about this? The hammers overall health is great and the other head is totally fine. Will this fix itself or should I do something about it?
 
Get rid of the aiptasia that is next to it. That might be stinging it. I can't really tell if the flesh is receding or if the polyps are just retracting.

That is a small clam of some sort on the side of it. it’s not touching the flesh. Should I remove it?
 
I'm talking about the second picture. The thing to the right of the hammer with the tentacles.

It’s not an aiptasia. I have never been able to ID this thing. it has never been a problem before. I don’t think.

It’s not recession - you can see flesh covering the side of the skeleton. Something may be annoying it.

If it was recession you would just see bright white skeleton - I’ve been there a few times, mainly due to high nutrients
