Foxface passed, Any advice?

I got worried yesterday and started doing extensive research after seeing white spots on the side of my foxface. Eventually the spots came off (sand on slime coat)
After reading on several threads that they shed a slime coat after being stressed I calmed down a little bit and decided to wait til the morning.
However this morning during feeding she did not come out of the rocks. I decided to flush her out with a little current from a baster because she wasn't moving.
She swam out and laid on her side breathing heavy, I immediately put her in the QT, She was gone in a couple hours.
First picture is the white spots. Second pic shows a large white patch that's only on one side with little spots.
I'm thinking parasites, but would like more knowledgeable feedback.

In the second pic the black dots on the lower belly remind me of “black ich” (which is actually a type of fluke-like parasite). Is that an accurate representation? See link: https://www.reef2reef.com/threads/black-ich-turbellarians.259912/

The first pic looks like it may have had a blockage or, perhaps, it was just well-fed. It could also be internal parasites causing the large abdomen—- but curiously it does not look that large in the second photo.

Those white spots look like sand to me, as well. It’s possible they were ich but I’m leaning toward sand.

1) How long has the tank been set up?
2) What are your water parameters?
3) Did you notice any other symptoms? (Swimming in to powerheads, scratching, flashing, hiding from light, heavy breathing prior to today, erratic swimming, etc).

I don't see any signs of Ich or Velvet. Velvet would still be a possibility though since it can infest the gills heavily enough to cause death without much visible symptoms.

I would say the more likely cause was a gram negative bacteria infection based on the discoloration and markings. The markings could also be caused by the stress from something else impacting the fish so it isn't a sure thing.

Unfortunately, I doubt it is possible to find an exact cause without a thorough necropsy.
 
Any recent additions to the tank? Fish, corals, inverts? Anything wet...

Tank is 6 months old. 70g w/ 20 sump
Params. are good (mag is a little low) Testing everything twice a week.
It wasn't swimming directly into a power head but would occasionaly swim in the current bouncing off the front glass.
She was hiding in the rocks a lot for the last couple days.
the spots came off within an hour of me noticing them.
I had moved a couple of rocks at the top of the tank for better coral placement, which I think stressed her out and probably put the sand on her.
I'm mostly worried about the other 9 fish in the tank.
2 clowns
2 Banggia cardinals
2 Firefish
1 Rainfod gobi
1 two spot gobi
1 Marine Betta
