I have a foxface and I know it can pack a little punch and he eats right out of my hand doesnt mind if i am in the tank.
Question is Does he have to want to inject the venom or is it alway there ready for whomever touches it?

You can tell. He will make the spines extend and fan out. If they are out they're ready. They really have to feel threatened for them to do that. Mine only extends them to the purple tang in the tank. He eats out of my hand also. For as skittish (sp?) as they are, they can be very bold once they are comfortable with you.

This is the way I have thought it was as well , but I am trying to clarify , When I am in there the fish doesnt flex (as I call it) but I know people who have bumped lionfish before that were docile as well.
I will be thrilled to get some more comments! But so far 100 per cent think they need to feel threatened. Does anyone want to participate in a trial?

The venom is in their spines, they can't turn it on or off. If you poke yourself with their spines, whether he is bent out of shape or not, you will get stung.

Actually it's not in their spines either, it's in the venom glands at the base of the spine. When the spine is compressed, it pushes down on the gland, and the venom is forced up the shaft.

I have been stuck by a foxface before a couple of times. He was just too friendly and I was careless. It wasnt intentional. I had a very mild reaction with minimal pain and no swelling. If you are allergic to bee's its a similar venom and could be dangerous.
