I was wondering what the beat way to frag mushrooms and what way will give it the best chance to Survive?

Clean surgical knife. Cut through center of mouth. Make sure you get some foot.
Dip cuts in iodone when your done for 5 minutes.
Put on rubble or sand in container in low light/flow area for a week or 2 and you should be fine.

Can I cut a piece off the foot instead of cutting through the mouth
 
yes you can cut a piece of the foot with some of the top. I fragged my frank in 4 and a tiny piece of foot was left and grew into a new one

Yes you can try. My shrooms leave a little piece of foot behind when they walk around and that piece always develops a mouth.
You would better your success getting a piece of foot with mouth and tips the odds in your favor.

You pretty much can't fail using a surgical grade blade. Giving them a quick iodine dip and putting them in a container with low light and flow and letting them heal.
I dont think I've ever lost a frag following these rules stricly.
 
Make it look like Pac-man :) (seriously)

Twice I've had some smaller rocks shift and a large shroom was attached to more than one rock. The first time, I tried to intervene and it was literally ripped in half. Both halves lived. Second time, the frag rock shifted and gravity did the work.
