If we're talking custom stuff:
Non-aquarium related, something on the order of the Jaws or Jurassic Park art would be awesome... I'm sure we all have a few ideas for designs that would look fantastic in our homes, but that are so specific that we're unlikely to find it in a store. For example, my wife and I met thanks to the Final Fantasy games, and I can think of a few designs (FFVII title image, for example) that would look awesome as a 3D artwork with light-up elements.
Aquarium-related, some kind of equipment cabinet with an integrated top-off reservoir would be pretty cool. I need something like that for my nano tank... A place to mount my GHL controller and route all the cables and wires through, store a few supplies, and hide my top-off container. I've jerry-rigged an end table to work for that purpose, but the cabinet in it can barely hold a 2-gallon bucket, and the wires and such are just kind of crammed into it. Having something that's actually designed to work for that would be much nicer. =)
And then I'm currently working on an upscaled version of that for use as a mixing station/work station/supply cabinet for my larger tank. Large cabinets on that bottom that can hold two 20-30 gallon water tanks, a nice counter for fragging, acclimating, cleaning, and any other kind of aquarium-related work, and then some bins or cabinets up top for storing my way-too-many supplies.

Seriously, thanks for doing this and for soliciting input! I have some of your stuff (including most recently the Apex toggle box you do with Harry's Aquatics) and I love it all! I'd second the person who essentially said a gift certificate for a custom order. You're one of the very few places to get super high quality custom work done for serious reefers, so why not showcase/provide exactly that? For instance, I'm sinking an ungodly sum of money into a new custom sump setup. I'm getting everything done in gray and orange---and the only thing I can't seem to get made is a skimmer in that color scheme. If I had a $1000 gift card for custom work from you, I'd order that in a heartbeat and post all over R2R about it!

I think a canopy/light shields of sorts would be cool. Three sided shield that rests on the lip of the tank. The expanded PVC would be super lite to move, yet block that nasty spillover light. Then add some of those sick light up displays or incorporate a controller display mount....

Done! I ship worldwide already so that is easy.
I'm not 100% sure either...
My job is kinda odd I guess as I just build stuff. What that stuff is can be aquarium-related or not. I do a lot of movie stuff and work with bigger brands like Samsung and Edifier and so on.
I think I am like BASF I may not make all the products in the picture but I make them better. Well, I built most of what you see in the picture excluding the chair.
