So, today I noticed my two head frogspawn was one... then I noticed the polyps sitting on a rock 6" away... is it a gonner, or can I glue the polyps to a frag plug and it grow a new skeleton?

I don't have them but from what I know it will be okay. I've read your best bet is to put it in a shallow dish with rock rubble to attach. Of course you could probably glue it to the plug but personally I like to get my frags on a small rock and then on get the rock onto a frag plug

The condition you have is called polyp bail out. Your tank condition is bad, the coral cannot live there and "bail out" hoping to get to better condition.
The only hope your the polyp and the rest of your tank is to get condition of your tank better.
Good luck

Yep ! Happens to all of us. It is now a dead coral decoration. Just one of many to come.

My frogspawn frag done the same, due to increasing Calcium to quickly, I left the stalk and after a few weeks I saw baby hears growing from the stalk.
