Galeaxa dying?

The left side had lost polyps? And the peppermint shrimp and cleaner shrimp seem to be picking in this area now. Are they killing it or is it dying on its own?

Probably dying on it's own but the shrimp won't help it recover.

What are your water parameters?
 
Temp 77.5
Salinity 1.025
Alk 9
Calcium 480
Phos less than .20

Running GFO and carbon in 2 seperate reactors.

All corals and fish look great. Nem looks great also.
 
Lighting conditions in which its under? Have you moved it lately?
 
You may need to move it up some. Would all now depend on what it was under when you got it and how long its been under 40% LED.
 
Sounds like the light is a trade off. I'd bump it up some. Everything else doing fine? This the only LPS you are dealing with?
 
Everything else looks great. I have bought other stuff from this store always had good experience with them.
 
I've had them die off myself, with it not affecting other corals. Got me stumped, but the lighting is a place to start, that and flow.
