Gem tang with white spots.

Hopefully someone can help me ID. Not sure if it’s flukes or ick?

Yep, looks like ich to me too.
 
Flukes generally will not cover the body in this fashion and it is ich based also on side of nodules, and needs to be treated in quarantine at your soonest. Best treatment would be coppersafe or general cure with coppersafe my choice at therapuetic level 2.25-2.5 monitored with a reliable copper test kit such as Hanna brand, not Api
Also monitor ammonia level with a reliable test kit during treatment and would not be a bad idea to add an air stone for added oxygen during treatment
You will also need to leave display tank FishLess (Fallow) for at least 6-8 weeks to assure the cysts and trophants have completed their life cycle without a host and have died off

I have wrasses in there and I’m just not sure I can get them out.
 
There are lots of threads on how to manage ich in a DT if your interested in it usually some form of UV and high nutrition and pristine water. Plus others.

joke time - yeah all gem tangs have white spots that’s their natural coloring silly goose!

I have wrasses in there and I’m just not sure I can get them out.
It will never go away left untreated. Will the fish survive and appear to live a happy life? Maybe…. There are ways to manage ich opposed to treating and eradicating. However with ich management additional stress could cause the fish to pass whereas without the ich the fish may be able to fight off whatever stress is prevented (ie: power failure, heater malfunction, aggressive tankmate, poor nutrition, overcrowding etc…
 
Looks like Ich. I also see some on its eyes I don’t think flukes get on the eyes. but i’m sure if that’s wrong someone will correct me.

I have the same thoughts. I feel taking it out would cause more stress and possibly kill it faster.
It’s still pretty fat … hopefully it will make it thru the night and I’ll have to decide what to do in the am.
I work from home and feed pellets throughout the day with frozen at night.

Although will be meet with criticism I decided to qt the gem and treat with copper.
no other fish are showing signs of ick. I have a few smaller fairy wrasses that are going to really tto catch with out taking out all my rock… and I have some collector piece corals that I am not willing to risk.

Again I know this is an unpopular decision and I know this will not cure the tank of ick but for my own sanity and the wife not killing me it’s the best choice I have.

To mitigate future out breaks I plan on continuing to feed heavy both meat and pellets soaked in selcon.
Gem is in at now with a new friend and eating well.

lesson learned … qt tank will be a requirement for the future upgrade plans.
 
Forgot to mention I am treating with Cupramine reading the directions I need to be be around .06 mg/L copper.
i thought I wanted to be much higher? Or is it all based on what brand you are using?
I did buy a Hanna copper checker to test with.
What should be my target levels for cupramine?
 
Add the gem back to the display tank. Been in there for a few days and I see 1 or 2 spot but I’m trying to resist doing anything but feeding and keeping her happy.

I did notice a budge on the side of the gem now. Positing some pics to show you all. Any ideas what it could be? I have been keeping nori in the tank at all times and feeding on the heavy side.

I am no expert, i have seen spots like this on the kole tang. I believe them to be scratches from rocks, and the few times ive noticed theyre gone next day or two. Keep eye on it but nothing actionable. I would say be happy and glad the fish made it thru treatment. Is healthy looking, keep it eating good nutritious food and run powerful UV in mean time
