every two to three weeks which helps assure the bacterial levels can adapt to the new loads

I am sorry but there is no rule of thumb on this. If adding a 2-4 inch fish throws off your bio load
Then your tank isn’t ready or something else is horribly wrong. A fully cycled tank should be able to handle adding several fish at a time, You shouldn’t have to wait 2 weeks between adding fish

now you said your fish are dying after adding them
I will say unless your tank is very new or heavily stocked your fish most likely didn’t die from ammonium poisoning
Anytime your buying new fish and adding it to an established tank it’s difficult even for season vets

that new fish can add parasites and diseases
That new fish will get picked on by the established tank mates because that tank is already theirs - so make sure you add the tougher fish last
3rd that fish is so stressed it’s immune system isn’t as strong so your tank can give it parasitic and diseases-
4th you might of just bought a bad fish from the pet store that wasn’t eating and slowest dies
