Goby "Coughing" Need help

Hello all. I've been fighting what I believe is ich on my clown goby for several months now. Today, after a change in treatment (going from hypo salinity to TTM) he started to get worse. Now he is very lethargic and "coughing" (video link below).

Here is a rundown of the symptoms, some history, and last 72hrs.

- Flashing and salt specks lead me to believe he has had ich. Originally treated with Marine Land All-in-one. Symptoms went away but came back. Next up I tried 3 weeks of hypo (first week included marine land all-in-one) at a salinity of 1.008. 95% of spots were eradicated but he still had 1-3 at a time and would flash. So last night I prepped a 1 gallon tank (he was originally in this tank but I moved him to a 5g for the hypo treatment). I had cleaned this 1g tank with vinegar and rinsed 3 weeks prior.

- 1g tank has a heater, bubbler, and half pvc pipe in it.

- Last 72 hrs. Over the course of 48hrs I raised the salinity in the 5g tank he was in from 1.008 to 1.012. 24hrs ago I transferred him into the 1g tank with a salinity of 1.015 treated with an ammonia reducer.
- He wouldn't eat this morning and I noticed him coughing. I was nearly out of RO and salt water so I transferred him to a small cup with matching salinity with the water I had remaining and stuck in a bubbler. Ran to my LFS and got water. Cleaned his 1g tank again (with RO). Refilled and matched salinity. Added him back. No change in symptoms after 1 hour.
- Gave him a FWD. He was upside down the entire time and breathing heavy so I cut it at 4min. I didn't notice anything come off of him during this time. Symptoms did not improve so here I am with this post. It's been a stressful day for the little guy. Any help would be much appreciated.

@HotRocks @4FordFamily able to give any insight? I encountered "coughing" with another fish I purchased from my LFS at the same time as this guy. The goby showed signs of ich (no coughing) a couple days after I got him so he has been in QT since then. Within 10 days the other fish (mandarin) started coughing with no other symptoms and died a few days after that. The goby just started coughing today. I wasn't able to get a diagnoses for the mandarin from the forums so if you have any input I'd greatly appreciate it!

It sure looks like flukes. Do you have formalin? Tattered fins lead me to believe maybe you have encountered a hypo resistant strain. Which would also be indicative after none dislodged in FW Dip.

Could I have been doing something in the other QT that kept them at bay for so long? He was in that QT for well over a month and only showing signs of ich. Could he have had PH shock or something not related to an illness?

edit: I moved his body into a cup to have a closer look. I noticed reds and greens inside him as well as what looked like pepper flakes on his body which seemed to be falling off into the cup. Could black ich kill that fast?

Well I think the biggest issue is that he wasn’t eating, his stomach is very sunken and he was emaciated. I suspect flukes as well. Sorry for your loss :(
