NO new fish introduced to the tank for a year+, wondering if my bullet goby had ich or sand, fish swims, eats, acts normal, no scratching and don’t see anything when light are normal just when high blues at night. Wondering if I should take him out

If it’s sand a few grains should come off while it’s swimming. Do you quarantine your fish? If not this could be ich. Ich can lay dormant in a fish until the fish gets stressed out and becomes vulnerable. This is how it can show up after a year even if you didn’t add anything new.

Should I take him out, or see if he gets better

That does look like ich.
What other fish are in the tank with it?
When was the last time you added a coral or invertebrate to the tank?
Removing this fish won’t help if it is ich, since the other fish in the tank have now been exposed.
There are a few other conditions that cause white spots on fish, but this does look like ich.

Agree on ich and all fish should be quarantined and treated with coppersafe with at least 2.0 treatment level and tank fishless for at least 45-60 days- but you can leave any inverts and coral in the tank safely allowing any protozoans to die off without a host fish

Unfortunately you’re going to have to take all your fish out and quarantine them with copper and leave your tank fallow for 2 months or you’re going to have to deal with ich for a long time.

You will want to get a second hand tank from craigslist or thrift store or a starter tank kit from walmart which has most of the essentials
