Gold rim dying with no explanation

My gold rim has been doing great. I have had him for about six months. Yesterday morning he was eating and acting normal but today looks like he is on his death bed. He is in a 90 gallon qt tank/holding tank while I am setting up a 310. He has been through 30 days of copper and three weeks of prazipro2nd treatment. All medications stop about two months ago and he has been doing fantastic. His tank mates are a coral beauty and a orange shoulder tang. The orange shoulder is about the same size as him. I have seen no signs of sickness and he had a great appetite. I just can't figure it out. What could take him out so quickly?

Any chance it's a water quality issue?
The tank looks cloudy and COVERED in cyano.

I would suggest a water change, make sure there is enough oxygenation in the water and they require lots and lots of flow and a large swimming area.

I just added a power head to the surface to get more oxygen if that is the problem. All other fish are active and acting normal.

I had an issue with a Morish idol, who was in qt with 3x Anthias. I put the air stone in the water the day I got the fish, they went in the qt that was pure clean, untreated water. The MI was a really good healthy size and was in the LFS for months on hold for me. He died over night in the QT, yet the Anthias were fine.
The only thing we put it down to was lack of oxygen with the water. Now I put the air on a week before the fish get go in. Different fish will react to issues in different ways and time.
Definitely add more air. Have u checked nitrite, seems they have been in there a good period of time?
run some carbon to clear the water up, might help

Nitrite is 0
I do have carbon in a hob power filter but I will change it and add some new.

He is still on the bottom of the tank. He has moved a little but still struggling. I have seen him moving a little more but maybe I'm just being optimistic.

Cyano. Im sure the led shop lights I have above the qt tank is a contributing factor.

Could be contributing to lack of oxygen if that does prove to be the cause .

Tested everything except phosphates

Additionally, when you stick your finger in the tank, any stray voltage?

Are you using RODI or tap water from the faucet?
What test kits are you using ?

Are you able to add oxygen via airstone to tank ?

I would try to siphon out the cyano and do a WC with some new SW. That large of a mat is not a good thing to have in the QT. I would also reduce light to just ambient light for the remainder of the QT time. No ned for much light over a QT. Just enough to see the fish and no more.
