Gramma in trouble

I have a royal Gramma that used to be very active. These days it was hiding in the back and finally it came out (not looking very good) . It's barely breathing and looks like it can't move. The surface of the body also looks like an old car with chipped off paint. (Faded white blotches on the purple)
I've isolated it to a small section of the tank to see if I can treat it some how. Any suggestions?

I’d recommend getting him to a separate tank. Use API Furan 2 and Kanaplex. Looks bacterial.

When my gramma ran into the glass then lost color, blotches on body, and frayed fins. He laid in this position for a week didn’t eat:

This was my thought as well. I’d also add metroplex to the mix, treat for 10-14 days.
 
Did a fresh water dip and ordered meds online (local Petsmart didn't carry them).
Sadly he didn't survive. RIP. -_-
