Hammer broke off skeleton and frogspawn not opening

Why would my hammer break away from a portion of its skeleton? Is there anything I can do to help it at this point?

Also I can't seem to figure out why my frogspawn won't come out. I've tried several different locations and lighting but I can't seem to pin point the problem. I do give all my corals coral frenzy about twice a week. Any idea why it doesn't want to come out?

If you are referring to the flesh portion coming away from the skeleton. It is regressing or not to well. What are your water perimeters?

A healthy polyp of euphyllia will have the skin wrapped around the outside of the skeleton. It will also go down the skeleton an half to an inch beyond the crown.

Alk- 9 dKH
Calcium- 450 ppm
Mag-1300
Salinity- 1.025 ppm

I did a water change I think it was Wednesday last week?
 
Nitrates , PH and phosphates are also very important. Regression of euphyllia or basically any LPS can be gradual. It may look nice and healthy for months before you see the regression. When that is the case it is generally the water is not to their liking.

Can you see flesh on the skeleton of the other polyp? It should look as though the flesh is wrapped around the skeleton stalk. Not just sitting atop the crown.
