Hammer coral critter

I have a hammer coral that seems to have a parasite and I need some help on what to do I will post some pictures

Pics fuzzy but may be nudibranch- remove it or dip to remove it as a precaution
Also keep hammer off the sandbed as sand can and May irritate them
 
I don't see any pest but the tissue is dying and breaking apart. White stringy things sound like mesenterial filaments from inside the coral, which is what I think you are seeing around the inside of the mouth that's partially gaping.

Tank looks pretty new. Can you post your water parameters and light level? How long have you had it?

fluval evo 13.5 gal
Everything is stock
2 clownfish
2 hermit crabs
1 nassarius snail
1 branching hammer coral
It has been established for about 9 weeks
Temp - 25 degrees Celsius
I will test my water tomorrow as that’s when the test kit arrives and I have had the hammer for about a week
