Hammer coral dying?

Idk why my hammer coral is dying. All other corals in my tank are doing great and I have had this hammer coral for almost a year now and now it's dying, why??
All my parameters are good and all the other coral are doing good.
I see my clownfish hosting it every night for almost a year now and I don't see that as a cause of it dying. Can y'all help me?

And also, I have seen some flatworms on it and sometimes it disappear and another day it appear on it again. Would the flatworms be the issue here??

And it is also not the brown jelly disease.
