Hammer coral with a melting head?

Hi people

Can someone help to see what is happening to the hammer. Seems like the head is melting away?

Is it some sort of a disease and what can I do help the coral

looks like a bail out to me if its just hanging off. could be alk swings or injury from fish.
 
Anyway. This is a photo when the hammer is fully opened. You don't notice anything. Most of my LPS are doing ok except for 2 acanthophyllia starting to recede.

Those numbers are great! How’s the hammer looking now? You could do an iodine based dip/bath for a few minutes if it’s not getting better.
 
Solid parameters! Although not necessary,, mine seem to do better with magnesium around 1400 but your magnesium is not the issue and you have a good amount of nutrients. Just keep it stable.
