Hammer dropping heads?

Hey all!
New to the hobby and I have a 44 head hammer that just dropped 2 heads overnight. They are not dead and seem to be ok. What could cause this? I had an arrow crab in there that I just took out because he was causing chaos and killed and ate one of my small cardinals. I thought maybe he might be agitating it?

i've heard of polyp bailout before , happened to me a few days ago. sometimes i feel like it's a response to stress or light changing
 
Bailout do to stress and rarely attaches and form new heads.
Check water parameters and too much flow also causes this .
