Hammer flesh

So I’ve had this wall hammer for 2-3 months now. And everything is fine I haven’t had any problems with it. When the lights are on it’s fully extended and puffy. But I just noticed after it receded when my light turned off that it’s some damage to the flesh and two little hammer polyps came off. I was just wonder if anyone had any ideas. But again it seems to be fine when the lights are on and I literally just noticed like 10 mins ago

hi ,let us see a pic when fully open,all params would help,fish ,inverts in tank list,keep eye out for something picking at :)
 
I would raise it so that non of the skeleton flesh is in contact with sand, it really irritates lps and can cause a slow decline in the coral.

Also flow can cause this. They like to "wiggle" in the flow, rather then being "whipped" about causing the polyp to get cut on it's sharp skeleton.

Here’s a pic of it almost completely opened because had to move a coral so it shrunk a little. But I have 1 clown, fire shrimp, a hermit crab they go nowhere near it, and a few snails that’ll get close to it. I’ll definitely take your advice on raising it higher. I also have a random flow generator that’s not pointing at it directly but get a good amount. If I could post a video I would. I wasn’t able to do my parameters right now I will post later just wanted to get a quick pic
