Hammer has a couple damaged polyps.

Hi just noticed that it looks like my hammer has a couple damaged polyps. What can cause this? I will attach pictures, thank you for any help.

Can you point out the damage?
How old is the tank?
Are they on the sand?

On a side note; you don't want zero no3, po4. May want to do something about that. What test kits are you using?
 
Sure here is a photo. I am using API for the ammonia, nitrate , nitrite , nitrate and phos. everything else I use hanna. My tank is 5 months old

The Tentacles are damaged but the polyp looks happy. Anything from a crab to flow can damage tentacles it’s hard to say why but it looks happy

My guess was the flow because that side was always open when I put it in because there is a powerhead on the back wall behind it and It was moving the water straight across and bouncing back off the front glass and pushing the polyps to the rear. I just adjusted the powerhead now to face more towards the side. I wasn't sure because I posted a video on a different forum and some people said the flow was fine.

Hmm, looks like the mouth. But, it is blurry.
If it's healthy it will heal itself. It looks healthy now. Just curious, how is the flesh of the head around the stalk? Should come down of the head an inch or more.
Don't forget to work on the nutrients.
 
I can't really see the flesh around the stalk but here are some more photos.
