Hammer lost head

The entire head literally just fell of the skeleton!! Weird cause even the head that fell of looks healthy? The entire head just sitting on the sand bed now. Looks full too. Not sure what is going on?

Will check water perimeters when I get home again but last I checked everything seemed fine. I did raise the temp of my tank by from 76 to 78. Could this be the cause?
 
Can you post a picture? The only thing I know of like that is polyp bailout, and in that case it definitely shouldn't look healthy or happy.

With the tissue appearing to be missing on that polyp it does look like polyp bailout to me, but the other heads look good.
A small temp change wouldn't have caused it.
