I have a green hammer that has been in the tank for about 9 months. Its about tripled in size from the single head polyp I got it as. Today I noticed a little "pillow ball" floating in the current of the tank. I am certain it was a polyp from this hammer that had somehow detached. Anyone seen this happen before and should I be concerned. It does receive some pretty decent flow where it is in the tank.

Not a great pic - with my cellphone, but here is one of them on the sandbed

I wouldn't worry about the occasional broken polyp. It's not uncommon to see every once in a while, but if you start to see more appear pay close attention to the coral. Sometimes fish can nip at the polyps and spread them around the tank that way in which case there is a problem.

If you notice the entire head floating around the aquarium, mouth and all, it's called polyp bailout and is a last ditch effort by highly stressed specimens in attempt to find a new place to live, hopefully less toxic. A single loose tentacle like in your photo is most likely from a physical abrasion. Your specimen looks good to me.
