Hammerhead coral polyps detatched

I have a hammerhead coral which has been happy for about 2 months, on checking the tank this morning the polyp part seems to have detached in one section away from the stony part of the coral.

Does anyone know why this may have happened and if there is anything I can do to try to save the coral?
 
This is known as polyp bailout which means that the coral in fact was not happy but in stress and did not like the conditions it was in.

Right now you could try and put in in lower flow and better conditions to help save the unattached part but that would be very hard.
