Hammerhead coral - so fragile?

I had this hammerhead coral for like 6 months, and it very good and expanded all time since i got, last two days i felt the size it bit shrunk or missing hammer tentacles, the base was stuck too deep inside sand and i thought i just bring it up front or attach to a rock on the bottom, so i took it out for like 5 sec and put it properly in place, during this move i haven't touch the coral itself, was just holding on the base rock, then i notice the green tentacles falling/floating around, there is atleast 5 or 6 pieces (see pic of some). Is it dying or did i do something wrong? or is it the effect of Coral Beauty angel i put in the tank couple weeks ago (that may biting it with out me even noticing it)

Right now you can see it shrunk IMG_0051.JPG IMG_0050.JPG IMG_0049.JPG to 1/4th of its size.
 
My hammers and torches are pretty hard. If I were to guess, I would say it's the dwarf angel nipping at it. You can try putting the angel in an acclimation box and see if the hammer recovers.
