I do much of what is already mentioned here.

I orient with the 10ml line, and I wipe down/polish the glass even between steps, every time I touch the glass. I'd be a crime detectives wet dream because I leave finger prints easily.

Another thing I'll do is after shaking I make sure I don't have any bubbles. If you see any bubbles stuck to the side, you can put the vial on it's side and spin it slowly and they'll come off as the air hits them.

I think the lid should always be closed to make sure the light is consistent.

For those tiny bubbles on the glass you can also tap on the table to loosen off of the sides.
By the way, don't rub on the fill line too much - I eventually managed to rub the line off of one of my tubes.

This is exactly why I use a syringe now. Surprisingly, even though the line rubs off, the Hanna Instruments label never wears off. I have new ones, but only replace them when they get scratched.

BRS has them in a 4 pack, and a 2 pack.

The 4 pack is way cheaper, but they don't come with caps.

$10 for the 2 pack with caps, $22 for the 4 without caps. Wow they went up! Think I payed $15 for the 4 pack a few months ago.

I sometimes get a really high phosphorous reading (about 20 times normal) on my Hanna ULR phosphorous tester. From the sounds of it, you are using the phosphate detector. I think I get this kind of reading if I get a particle in the sample that is phosphate rich. I have done a follow up reading a couple of times when I get a high reading and it is always far, far lower.
