It came very discolored and this is the best it has been since I got it shipped to me. I’ve fed it lightly around with reef roids and it’s been in 3 diff spots. The coral just never finds a happy spot . I’m running hydra 32 with 60% uv , 70% blues max. With like 10% white. It’s probably in 100-150atmThat is one unhappy coral.
Looks receded and bleached.
Need to get it off the sand. Need to give it moderate flow. What lights are you running? Do you know par? I would put it in about 150/200 or so. See if it heals up. Do you have reef chili or something like that? It will grab the small particulate. Tank feed, don't force it.

Po4 is high for your n03, try to lower it to around .05- .1
You want your nitrates 10-20 times your phosphate.
What are all of your other parameters and how old is your tank?
That's interesting. Could be some sort of boring or burrowing creature. They are fairly common on wild colonies actually and usually do not cause trouble. It may ir may not be related to your issues. There isn't really anything you can do if that is the case though.
