Hatian dying?

Not sure if this is the right area but can someone please tell me what a Hatian anemone looks like when it is dying? PH is through the roof, ammonia is up, nitrates and nitrites are up after a 50% water change yesterday. Yesterday my lemon peel was found at the bottom of the tank breathing heavily and died a few hours later. My saddlebacks are ok but this morning my xenia, hammer and frogspawn frags look horrible and all the numbers are back up. Only other things in the tank are a cleaner shrimp, large carpet anemone, two emeralds and snails.

Hatian has had tenticles turning almost black lately. It still eats but not really with the zest it used to. That is why I suspect it.... that and I can't figure out anything else in the tank that could be tanking the numbers like this in 12 hours.
 
I pulled its rock out. It didn't smell. I did find a relatively decomposed piece of silverside on the back of a power head. Couldnt see it until I was moving things around, its a powerhead under the overflor box so its not as visible.

This possibly my culprit?

Carpet is sticky and reacts to the touch.
