Having problem with hammer coral

Hi all,
Having problem with my bicolor hammer at least 30 days. Latest condition is in 1st picture. In the 2nd picture you can see previous condition.

I have tried different flow and light locations for 5 days each. Also performed dip.
Water parameters are as below,
SG 1.025
Nitrate 5-8 ppm swinging
PHosphate ~ 0
KH 7 dKH
Ca 420
Mg 1380

Other tank mates have no significant problem; frogspawn, torch, brain coral, acans.. and zoas..
Looking for any kind of advice.

Seachem reef dip
Pair of clowns, firefish goby and watchman goby , cleaner shrimp
Havent seen any of them bothering it.
 
Inspect for any pests by removing it and looking around the skeleton?

Shrimp can be opportunistic feeders, I'd check the coral at night, a few hours after the lights go out and see if anything is bothering the coral and look for pests. Reef dip is a good choice.
 
I have 3-4 different kinds of Hammer and Frogspawn... They thrive and grow rapidly in the same (dirtier) nutrient richer water my Zoathids, Acans Blastos, Chalices and others seem to like... Are you trying to spot feed yours? I have never had luck with it as the slim strings get blown off or the fish eat them before the Hammers and take anything in... The parameters you show state zero Phosphates... This could also be its problem?
