Clearly something is off, but it's really hard to tell what is actually on the fish and what is a product of the camera/lights... Can you get a few pictures with some white/standard room lights, by chance?

That's a juvenile Caribbean Blue Tang (Acanthurus coeruleus), not a Yellow Tang. The small spots look like ich with possibly some lymphocystis on the anterior portion of the dorsal fin. The blotchy white areas are likely due to stress.
:( any suggestions on what to do?

:( any suggestions on what to do?

Start simple.

At the beginning of October, I purchased a Puffer and placed him in a QT tank. Glad I did; Ich showed up.

What worked for me was a fresh water dip daily for 7 days and dropping the salinity in the QT.

I gradually drop the salinity down in the QT to 1.012 and gave the little guy a 5 minute fresh water dip for 7 days. Additionally, I ran an air stone in the tank at a high rate to provide extra oxygen.

After 7 days, monitor him to see if there is a relapse (mine had one). So, I started the process again. But this time around, it worked. I haven't seen any signs of Ich in over a month and he is eating well. I'm SLOWLY bringing up the salinity now.

Of course, most people will tell you I was fortunate. You may require a more aggressive form of treatment if dips and hypo-salinity doesn't work.

I’d recommend copper in a qt and agree, that’s not a yellow tang, it’s a carribean blue tang. They get very large and turn progressively more blue.

It may have velvet as well but all I can positively identify is ich spots. The color blotchiness is often caused by velvet damage in the gills.

Agree with the above. Here's a link on ich and some options for you, but note that since you have a high likelihood of other issues, I would strongly recommend against the "ich management" course of action. Note that this means that you will want to leave your tank fallow (fishless) for 76 days. Feel free to post any questions you have - we're here to help you (and your fish) get through this.
