From the irregular size I would start with a FW dip to check for flukes. Has he had any treatment or any other symptoms?

Could be bacterial, perhaps secondary to flukes like suggested above.

He’s been through copper, Kanaplex, Metroplex, a Fw dip, and a Rally Bath 90 min
