I've had this guy in QT for about 2 weeks and he's been doing fine. Active, hearty eater, good color. However about 3 days ago I noticed his tail was a little frayed and didn't think much of it. Then a day or so later notice a little white around his gills, slightly concerned, but he was still very active and eating well. Tonight I can see the white has moved to around his mouth. He was swimming in the corner near the HOB filter and not swimming about. I was able to entice him out with some food he had maybe one or two bites of mysis and that was it. He continued to swim in the path of the food it was hitting him in the face but he just wouldn't eat. Now I'm really worried. Video below (might have to go full screen to see what I'm saying)

Can anyone help ID so I can treat before losing him!

Is he still in copper or medication? What did you treat with so far? Alone in QT?

The white around gills look like scarring, possibly HLLE. The tail issue could be bacterial unless there’s aggression.

If you’re treating with copper I would discontinue at this point by doing a 100% water change. Ideally, if you have another tank transfer to that tank and sterilize your current tank - after about 24hrs - everything needs to dry you can put the fish back.
 
It looks like a bacterial infection. It appears like the bacteria started on the tail fins(unless another fish nipped him) and then the bacteria progressed into the skin. It appears that the bacteria maybe colonizing within the skin and creating a deterioration of the tissue.(filmy white appearance) All the symptoms you shared are symptoms of bacterial infection. I’ve used Seachems Focus mixed with Seachems Kanaplex applied to frozen fish food. I’ve used this method in my display tanks without any issues pertaining to corals, other fish etc. please note I’m adding these medications directly to the food. I’m not using them as a method for treating the entire water column. Focus acts as a binding agent in combination with Kanaplex to stick to the food.
 
Thanks for the feedback. This guy has been alone in QT and I haven’t been treating with anything just observing.

Sounds like a bacterial infection, any other treatment options or are the two SeaChem products mentioned above my best course of action?
 
With the fish being in a QT tank you could use kanamycin at 50-75mg/L of aquarium water for 7-10 days redosing every 3 days. This medication is absorbed through the fishes gut so it’s fast acting. I mentioned the use of Kanaplex in my first response. Kanaplex is just a blended kanamycin based medication and when added to the food it’s end location would be inside the fishes gut.
