Help for Achillies.

This guy has been through copper at the store, hypo, and TTM. I don’t think this is ich. Spots are isolated only to what is seen in the pic. I was hoping it would fight off whatever is causing it but the skin around the white bumps is starting to get a little raised. Tough to get a great pic. And meds that would be recommended?

That could be ich. It could also be flukes. Have you given the fish a 5min freshwater dip yet?

I would do the dip, if no flukes come off I would re-treat with copper. If you confirm flukes you can treat the tank he is in with Prazipro. Either way all fish he shares a tank with need treated.

Did a fw dip when it arrived. I’d be surprised if it was ich. I have a powder blue, blue tang, and morish idol in the tank that have no spots at all. All were introduced at the same time. I was thinking maybe some type of bacterial infection. Tough to see in the pics but the area around the bumps is raised and pale.
 
Everyone is going to say to remove the fish and treat.

How big is your tank? Are the tangs battling? If so, try putting some plastic mirrors up on the outside of the tank. They get so interested in their reflection it helps reduce the stress on the fish. Feed them a lot of nori on a couple clips. It gives them another thing to do besides look for a territory to protect.

No fighting at all. 210g tank. Regardless of what I treat with I will do it in the display. I have no inverts and it’s a new setup so I’m not really worried about if treatments are Reef safe. Carbon and time will take anything out that I treat with.
 
For meds I have on hand: kanaplex, general cure, prazi powder and liquid, and ciprofloxacin

White-ish areas like this should be watched closely. This may well be the beginnings of a bacterial infection. Treatment would be in a QT with a wide spectrum antibiotic like Kanaplex, Furan2, Triple Sulfa, NFG, for 10 days and up to 14 if needed. If the infection goes sideways and much stronger treatment is necessary, the Trifecta of Kanaplex + Furan2 + Metro can work well in difficult cases.
 
I had a blue tang like that, didnt see any fighting at all, was a pain to get him out of tank to treat. Eventually the problem was fighting during lights out for sleeping spots. Stop after a month. Also had a fire goby with 1 translucent spot and whiter area around the tail, turned out to be flukes and treated with prazi. I also did fw dip too but sometimes they dont come off.

I’m likely going to dose prazi and either metro or kanaplex to the display if it looks worse tomorrow
 
If u have corals or anemone dont use prazi in DT. Took my corals a month to recover and my bta over 2 months to recover. He shrunk and turned white.

That’s what I’m going to try. Need to order more kanaplex though. You don’t know of any source to buy in bulk do you?
 
Update: the white area has returned to normal but the white raised white spots remain. They have now extended a bit all concentrated just above or below the lateral line. Also only on the left side of the fish. The right side looks perfect. I treated with general cure so far. I can’t manage to get it to hold still for a picture. The lateral line also looks a bit puffy if that makes sense.
 
Managed to get a couple somewhat decent pics. The areas that look like spots other than along the lateral line are actually on the glass.
