My yellow tang went through the copper treatment for ich back around Christmas and the tank was fallow during the treatment period.

I started noticing some fuzzy spots on the side fins 1 or 2 weeks ago. I didn't really think it was ich through, just because they were more fuzzy.

Tonight - I noticed it has spread to the top and tail fins. Nothing on the body.

Am I looking at another battle with ich again (twice within 6 months)? Argh! I'm almost hoping its something else. Only other fishes in tank right now are two clown fishes. Nothing on them yet. Just want to figure out if this is ich where I need to get it out to start treatment or whether it's bacterial and will take care on its own with some Selcon/garlic or something that I can work with in the DT.

It looks like it could be ich. How long have the spots been there? You might wait it out and see what else happens with them. If they get bigger and don't drop off after a few days, then it would be lympho... though I don't think it's lympho.

The ones with the arrows were the original ones that I saw back on the 13th (Could have shown anywhere between 3/10 - 3/13 - just know I took the pic on the 13th). Doesn't look like they've grown - but they're still there in the same spots tonight 10 days or so later.

The new spots I saw tonight could have shown maybe a day or two before if not only from today.

If they are the exact same spots from the 13th, then it can't be a parasite or worm. It is most likely Lympho.

In that case - the ones on the left side fin are lymph; the ones on the right side fin look very much like lymph (last picture I posted).

It's the ones on the top and tail fins that I'm hoping are not ich (for a combo lymph/ich). But I guess I'll know in a few days.

Still no known treatment for lymph? Just good water and Selcon/vitamins?

Yup; it's a virus that goes dormant and then will become active again. Probably when the fish is stressed and their immune system is lowered.

Well - It's looking more like lymph than ich. But he's getting a little torn up I think by the cleaner shrimp. Anything I can do to help him?

Looks like some infection is setting in where the cleaner shrimp has tore off his fins.

Are you soaking the fish food with vitamins to give his immune system a boost?

Just Selcon. Not sure what else to use
 
I'm guessing that at that point - I'm going to have to get him out of the display tank and into a hospital tank? (not sure if Kanaplex/Focus is ok in the display tank with some snails, cleaner shrimp, and 3 bubble tip anemones (and two clown fishes).

Using Focus makes it reef safe because it binds the medication to the food and prevents it from leaching out. However, if you wanted to QT him so you could dose antibiotics directly into the water that would, of course, be optimal. :)
