Help identify sick grouper??

My panther grouper is very bloated and has a small circular sore on his side I can't tell if it's a parasite or a wound.? Not sure what to do or what to treat with.

Also this is a 75 gallon, the nitrates are A little on the high side since this is a predator tank. But I've been keeping up with weekly water changes and everything else tests ok. 0 ammonia

I've had him for about 6 months

Very possible. If it has been like this for a month, I'd say that makes a gas bubble more likely.

The only other possibility I can think of is that he swallowed something he can't pass. Like something with a shell i.e. snail or hermit crab.
