Help IDENTIFY this dying frag please

Got this frag a few weeks ago, along with a few other frags. I am not sure what the name of this one is (image below), but it seems to be dying. The other corals are doing well. I have tried moving it around to get different air flow and light, but it is not helping. Nitrites and nitrates are at 0. Salinity is at 1.023, temp at 78degrees, and pH at 8.2.

What is the name of this coral and what are the preferred parameters / water flow?
When I first got it, the tiny polyps would protrude slightly from the holes and were very bubbly/ flowery looking..

+1 to Goniopora. You should increase the salinity slowly to 1.025 to 1.026 and try not to make your nitrates at 0. Some corals like to have little nitrates. Do you feed corals?

Reefroids and the tank has a good amount of amphipods/ phytoplankton
 
Reefroids do wonders also aminos would help so probably water parameters (make your own), moderate to low lighting and moderate flow ... I would put it between some rocks and have some type of wave mode (they love that). And I think key is leave it alone for a while. I agree with nitrates at 0.03 up to 0.06 and phosphates to 0.02 max My 2 cents
 
would submerging that in 3% hydrogen peroxide for 2 minutes then gently brushing it help to perk it up? It looks like it is getting choked out with algae. good luck
