Help identifying clown fish problem

Clown fish has black discolouration of the white parts of its body. This started with its tail and spread to body. Also has swollen abdomen on and off and had what looked like a parasite come off its body during a couple of treatments. Initially I thought flukes / worms so did a fresh water dip and API general cure in quarantine. It seemed to clear and I put back into tank after a month. Discolouration came back and what looked like a parasite or white growth middle of its body so I removed again and treated the same however saw no improvement second time. Thought if could prove it was flukes I would treat the tank.

The second freshwater dip seemed to perk it up for a little while and in the freshwater its discolouration on its tail lightened but did not dislodge any noticeable parasite hence I wondered if it was a stress discolouration. It is eating okay, it seems to have mild swim bladder issue particularly when it bloats which is on and off. It is eating a mixture of brine/mysis and marine green. Tried a couple of tiny marine pellets however bloat became worse. Today he has no bloated stomach. Any thoughts on what I am dealing with would be appreciated. Parameters in host tank all good with anemones, clams, Bangai Cardinal and coral all healthy. Quarantine is cycled so parameters are in range. Will take some readings tomorrow to update post however last reading had Ammonia/Nitrate 0 and Alkalinity at 8 PH 8.3. Salt did get on high side 1.027 due to faulty hydrometer in host tank but back to 1.0255 now. I have also taken a couple of pictures and will attempt to attach. Any thoughts would be appreciated.

I'm thinking bacterial infection. I'd get him back into qt and start treating with antibiotics. My recommendation would be to either use the "trifecta" of kanaplex, metroplex, and furan-2 or, if you can get your hands on it, nfg powder.
