Help! (Injury or Infection) what to do??

HELP! My flaco Hawkfish stopped eating about two days ago and started breathing heavily… Since then I’ve been watching everyday and night.. he has been hiding for about 4-5 hours and when he came out his side looks like this now!!!!

(I have been dealing with high Nitrate levels and been doing consistent water changes for about a week now to lower these levels.)

(3 other fish in tank are thriving and eating with no problem)
