Help Lost two fish already

Hello guys,

see attached photos of my sailfin tang. What do you guys think this is? I lost my Blue spot Jawfish earlier today and yesterday I lost my Kole Tang. I currently have all fish out of tank and in QT just don't know what to treat with as I'm still trying to figure out whats wrong. All tank param's are normal. Any help is great. Thanks again.

Update: Just found an anthia (attached photo cleanup crew was eating him)

How old is the tank? Size? Other animals? All tank param's are normal, what its normal?...
 
- Tank is 4 years old..120 gallon Display with 40 gallon sump mixed reef (Picture is sailfin in QT tank)
- Other livestock : 2x Black Storm Clownfish, Sailfin Tang, Kole Tang, Blue Spot Jawfish, 2 Anthia's, Red Star Fish, Coral Banded Shrimp, Cleaner Shrimp
-Current Tank Parameters
Temp - 77.8
PH - 8.1
Alk - 10
Amm - 0
Nitrite - 0
Nitrate - 8
Cal - 420
 
Yea I thought the same thing at first..but none of the other fish show those signs. The sailfin tang is about 5in. So the black spots/discoloration are a lot bigger than grain salt size. Might be wrong though?
 
They don't have to be white, could be scabs from when they fell off. Or can also just be discolored. every fish is a lil different in how they react.
Thanks..velvet does makes sense with the major fish loss so fast. I’ll get working on fresh dip than the other steps.
 
Sorry for your losses-

I would consider giving the sailfin tang a 5 minute FW dip, and then examine the bottom of the dip container for dead flukes (you may need a magnifying glass).
