Help me find captain whodunnit

I’m coming to you fellow reefers with a very strange lost of tissue. I’ve had this hammer in the tank for about 3 months and for the most part it was doing well. It filled up pretty well and then split. Up until yesterday everything was normal and when I came back home from work I noticed it right away. Whole head just gone, not a single tissue left. My parameters are- PH 8.1
Cal 420, Alk 8.5, Mag 1300, phos 0.01, Nitrate 10, ammonia 0, salinity 1.025 and I keep my temp between 77 and 79. I’ve also attached a picture when it was first introduced to tank and a torch in the same tank area. Any thoughts? I should also mention I have a flame angelfish and peppermint shrimp in there.

Couple years back I had a peppermint go from ideal tank dude to torch killer in the space of hours. Still no idea why. In the afternoon he was chillin as normal. About 3 hours later he was ripping up the torch like the Swedish Chef on a salad.
Don't know if that's common. Maybe someone else can chime in, but that was my experience.
 
I have heard they can go for euphyllia. I mainly had them in there cause I had an aptasia issue and they took care of it so I decided to keep them and feed them as well. If it was them they’re gonna have to go in sump.
