Help me ID this disease.

The black spots are too big to be black ich. I have no idea what they are. Any help would be appreciated.

Sorry but that does look like Black Ich. Especially the third photo in the list. Remember it isn't a small parasite like ich or velvet, it's a worm, more like flukes and treated with Prazi quite easily. A freshwater dip will give temporary relief.

Will paragard work, I have that on hand. If not I'll be going to buy some prazi
 
Seachem's Paraguard will work for some worms, but not especially effective on turbellarians for some reason.
Plus with Prazipro you can treat the entire tank (if one has it the others may too and may not be showing yet) as it is pretty reef safe other than hazardous for worms in your tank like feather dusters, bristleworms that may be in the rocks/sand.
