That would be a branching torch. I think k it's getting a beating by flow. Did the heads die in your tank or did you buy it that way. Most euphyllia like little flow but torches like more on lower to medium.
 
The pic with moonlight was in the store and the daylight is in my house. I just got it today :)
 
Those empty spots in the back does it eventually grow more in those branches ? U can see the empty spots clearly in the daylight picture.

Generally no. I have read where people said theirs sprouted a polyp. But i think the more believable is it can sprout a polyp but only if there is living flesh on the branch.

When new heads appear it is along the outer flesh that expands down the skeleton. A healthy euphyliia will have flesh outside the skeleton. A stressed one the flesh will start to recede and with no flesh outside only on the top.
