Help me save my Rainbow Welso

My Rainbow Welso has been in my tank for a month. It is receding mostly at the mouths. It still has good color but not as good as when he was purchased. I've tried moving him. I'm considering a dip but a few years ago, I lost a Welso and the dip was not helpful. Please advise.

Paramaters are good
Magnesium 1320
Phosphate 0.04
Nitrate 0.5
Calcium 420
Alkalinity 10.08

Mine struggled for three months,then I found out that my hemisphere were irritating it try setting it on a 1.5" to 2"
Pvc coupling. Worked for me ,mine looked like a skeleton. Also welso ime respond well to direct feeding ,I placed a large acrylic tube over him and turned pump off.keeps the fish away.

That is strange, have you noticed anything bothering it?
 
Nothing that I can see.
Our first welso died because we placed him directly on the sand, and a higher light area with to much flow. We did a little more research when we bought our second brain, we placed him on a piece of pvc pipe, in a low light area and almost no flow. Our hermits don't bother him, and if we see sand grains we dust them off with our turkey Baster. We also target feed mysis 3-4 times weekly at night. It worked and he's super happy.! Here's a picture. And he's actually usually bigger than this even.
