Help me save my torch!

No idea what's going on had a torch for about a month and all was well. Today it seems really ticked and has been losing heads. I tried moving it a little. Just did a 30% water change not sure what's going on.

Your water is way too clean , corals need some nitrates and phosphates to survive especially LPS , the coral is starving.

I've got quite a bit of diatom algae, I really don't think this API kit is right with my phosphates/nitrates.
 
Did you test your Alk? I agree that your water is lacking nutrients. Plus, I'm assuming that you tested after your water change. The coral looks like it has recessed some but polyps are still showing. The diatoms may be pulling nutrients away from your corals as well. Is your tank fairly new?
 
For future reference when looking for Euphyillia, I would look for branching not wall. They tend to be more hardier. Your picture looks like a wall torch. I agree with the above statements that your water is WAY to clean. I would feed heavier to raise those up some.A few questions regarding need to give you better answers would be:

How old is the tank?
Torches can be tricky, especially in newer tanks. Diotoms are also part of the new tank uglies. They should go away shortly.
How long have you had the torch?
They do Take some time to settle in.
What kind of lights do you have?
Looks like he is right on the sandbed. Torches like moderate to high light from my experience.
What kind of flow is in the tank?
Torches like moderate_Indirect_ flow. They like waving in the current.
Also need the following parameters if possible:
Alkalinity
Calcium
Magnesium
