I have seen several requests recently with people asking for help but, possibly through no fault of their own, aren't supplying much in the way of information. I see that in the fish disease forum there is this 'sticky', attempting to give guidance on how to ask for help, but I haven't seen one for generic problems.

I was thinking that it might be good to have a similar one in the other forums (or maybe just the beginner forum)? I have listed a few things below but will surely have missed something. Does anyone think this might be helpful?

Description of problem:
Tank (type and size/volume):
Water source (and change schedule):
Salt used:
Age of tank:
Rock used (dry/live):
Cycling method (if new):
Stocking list:
Lighting (type/schedule/PAR):
Flow (type and number of pumps/position):
Feeding schedule (incl food types):
Additives used:
Dosing schedules:

Parameters (including test kits used):

Salinity:
Temperature:
Nitrate:
PO4:
Alk:
Ca:
Mg:
Others as applicable (Ammonia/Nitrite/Potassium/trace elements etc):
ICP results (if applicable):

Photos (white light preferable):

Any other suggestions or alterations?

Looks good!
I’d also add ammonia and nitrite.

I've added an 'others' line. This is intended for problem tanks rather than just cycling issues (although they could be included too); plus I don't test ammonia or nitrite ;)
