There's nothing for ICH or Velvet that's 100% reef safe.

Unfortunately, everything that you treated with above (Treated her with prazi, metro, kana, furan, etc) doesn't really cover either one.

ICH and Velvet look very similar in the initial stages. At this point it appears to look more like ICH.

To determine if that's ICH or Velvet... can you tell us how long you've had the fish and when you first observed the flashing and spots?

If it is ICH, you would have to go fallow (fishless) for 72-76 days to rid the tank of ICH. Or you can just manage the ICH.

It is worth mentioning that most cases of ICH will complete the life cycle in about 2 weeks or a max of about 35 days. YOU COULD GAMBLE!;)

Ohh and I did 5 transfers of TTM lol for ich

Certainly looks like the tang has some sort of parasite (either ich or velvet). Honestly, needs to be pulled and treated for the condition.

Cant catch him man,

I tried to catch him for 50 mins day before yesterday

@Ahbaloch i know it stinks but you’re going to have to. I’m currently fallow for velvet myself because I broke my QT rules. I had to drain my tank and remove the majority of the rockwork in my DT to get it done. What size tank is it?

Yep...I've been there several times too. That's what makes you start following a strict quarantine protocol. Let me tell you...when you know those fish are free of all maladies...it feels amazing watching them swim around clean!

FWIW I eventually caught a hippo tang out of my tank with a fish trap. Wanted to change out two fish. The first fish was caught in the first 5 minutes but the hippo took probably 5 hours broken up over a few days when I had time to sit there. Every stinking fish in the tank would go in regularly except the hippo but eventually it followed another fish into the trap. May not be the best option but I really didn't want to go the drain the tank route to catch them and I wasn't in a huge hurry as it was just to change direction, not to act quickly to treat anything.

Fish trap?

Can yall share a picture

This looks like ich and likely an infection or two at some of the insertion points. On a few shots it almost looks like flukes as well.

What was your process with Prazi?

I agree, although hippos are so hard to gauge. They love wedging in the rocks. I would treat with copper in QT. Antibiotics (kanamycin + Furan-2) may be necessary as well
